A Subject Group's booking page is fully customizable. Rename the standard fields, turn the address and password prompts on or off, drop a short message at the top, and add your own custom questions (sport, jersey number, t-shirt size, coach name, allergy notes, whatever you need on shoot day). Custom answers can flow straight onto the session sheet your photographer takes into the shoot, so what a parent types at checkout shows up next to their child's name on the printed log.
Everything below is configured per Subject Group: open the Subject Group, click Edit, and scroll to Booking Fields.

The three screens above show the start of a booking: the family or student logs in (Last Name + Student ID or email and password depending on setup). After login, they reach the main booking form where the custom question show up immediately. Standard field labels can be renamed, your booking message appears at the top, with your custom fields appearing directly below the contact info. Required fields block the Continue button until they're filled.
In the booking form shown above: "Preferred First Name" and "Full legal Name" are renamed standard fields, "Please make sure to wear your team jersey!" is the Subject Group's Booking Message, and Team Name and Team Color are custom fields — Team Color is required, so the family can't move on without filling it.
After this screen, they pick a package and a time slot, then check out.
