You can customize how fields are displayed in the Client Booking Sequence in the Subject Group / Edit panel. It is a drop down in Edit Subject Group / Booking Fields:

You can rename the fields, turn on or off the password and address fields (location) and add a short message at the top of the first screen.  By default the "Show Location" and "Show Password" is not off.
